TMZ is reporting that Glee actor Cory Monteith’s untimely death at 31 years old was caused by “mixed drug toxicity, involving heroin and alcohol.” The staff of the Fairmont Pacific Rim hotel discovered Monteith’s body on July 13th during his stay in Vancouver, Canada. His death follows a history of substance abuse and a recent stint in rehab earlier this year. Details on the story are still developing.
